Job overview

Salary Range: $37,856.00 To $37,856.00 Annually

Starting Pay Rate: $37,856 Annually ($20.80 an hour)

Job Duties:

The successful candidate will be expected to:

  • Work with youth who are at-risk.

  • Complete intake assessments.

  • Provide information and referral services.

  • Conduct a youth risk assessment to determine their potential for crisis and involvement with the juvenile/criminal justice system.

  • Maintain regular contact with participants and their family members to monitor progress. This includes home visits with the individuals we serve.

  • Work with youth with developmental disabilities who have risky behaviors or who are at-risk.

  • Works with youth with developmental disabilities and their family to develop an individualized support service plan.

  • Maintains regular contact with participants and their family members to monitor progress.

  • Prepare and submit all required reports.

  • Network with Care Coordination Organizations (CCO’s), agencies, organizations, and schools to coordinate services.

  • Maintains regular contact with participants and their family members to monitor progress.

  • Valid Driver License required.

  • Other duties as assigned.

Education:

  • High School Diploma required; Associates degree preferred.

Knowledge, Skills & Abilities:

  • Advocacy and youth development preferred.

  • Mediation, negotiation, planning, community networking, innovation, collaboration, oral and written communication, flexibility, organization, public speaking, working with diverse population.
