Social Services & Admissions Director

Prestige Care Center of Nebraska City is seeking a compassionate, organized, and results-driven Social Services & Admissions Director to join our leadership team. This role is critical to ensuring smooth admissions, strong census management, and resident-centered social services from admission through discharge.
Key Responsibilities

Oversee and direct all social services and admissions operations within the facility.

Coordinate and manage the admissions process, including referrals, bed management, and pre-admission screening.g

Serve as a liaison with hospitals, referral sources, families, residents, and interdisciplinary team members.

Support census development and retention efforts through timely follow-up and relationship building
-
Provide resident and family support related to adjustment, discharge planning, and care coordination.n

Ensure compliance with state and federal regulations related to social services and admissions.

Maintain accurate documentation in electronic medical records and referral tracking systems
-
Address resident and family concerns professionally and promptly
Qualifications
-
Previous experience in social services, admissions, or healthcare administration, preferably in a skilled nursing or long-term care setting

Knowledge of Medicare, Medicaid, and managed care processes preferred

Strong communication, organizational, and interpersonal skills

Ability to manage multiple priorities in a fast-paced environment

Experience working collaboratively with nursing, therapy, and leadership teams

Bachelor’s degree in Social Work or related field preferred (or equivalent experience)
