Prestige Care Center of Nebraska City is seeking a compassionate, organized, and results-driven Social Services & Admissions Director to join our leadership team. This role is critical to ensuring smooth admissions, strong census management, and resident-centered social services from admission through discharge.

Key Responsibilities

  • Oversee and direct all social services and admissions operations within the facility.

  • Coordinate and manage the admissions process, including referrals, bed management, and pre-admission screening.g

  • Serve as a liaison with hospitals, referral sources, families, residents, and interdisciplinary team members.

  • Support census development and retention efforts through timely follow-up and relationship building

  • Provide resident and family support related to adjustment, discharge planning, and care coordination.n

  • Ensure compliance with state and federal regulations related to social services and admissions.

  • Maintain accurate documentation in electronic medical records and referral tracking systems

  • Address resident and family concerns professionally and promptly

Qualifications

  • Previous experience in social services, admissions, or healthcare administration, preferably in a skilled nursing or long-term care setting

  • Knowledge of Medicare, Medicaid, and managed care processes preferred

  • Strong communication, organizational, and interpersonal skills

  • Ability to manage multiple priorities in a fast-paced environment

  • Experience working collaboratively with nursing, therapy, and leadership teams

  • Bachelor’s degree in Social Work or related field preferred (or equivalent experience)
