Location: Bergen (3 days) & Essex (2 Days) Counties (Paramus & Bloomfield)

Onsite FT 40 hours M-F:9:00 am - 5:00 pm

Hourly: $24.04

Job Summary:

As the clinical care coordinator, you will provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning. Additionally, you will provide all services consistent with SAMHSA's fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness. This is a great entry-level role and learning opportunity for individuals looking to gain experience in a clinical and/or community-based non-profit role and to collaborate with social workers, clinicians, and prescribers to provide holistic and comprehensive care.

What You Will Do:

  • Engage consumers and their families.

  • Work with the Outpatient Department to assist clients and staff with coordination of care.

  • Provide linkages to community services and resources.

  • Provide services based on the tenants of evidence-based practices (i.e., IMR, MI and TIC).

  • Ensure consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role.

  • Communicate observations and information about the consumer in the consumer's service record and to team members.

  • Participate in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge.

  • Provide input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives.

  • Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collect information on consumers and reflects all demographic data.

What We Need to See:

  • Bachelor's Degree in Social Work, Psychology, Counseling or related field

  • Bilingual (Spanish) required

  • At least 1 year of experience in a mental health, behavioral health, medical, or another related field.

  • A valid driver's license and must be insurable by agency's automobile insurance
