Day Shift, 9:30am - 6:00pm Monday - Friday

Weekend Coverage: Colleague will work one weekend every four weeks

Holiday Coverage: Colleague will work two holidays per calendar year

This position is based in the House Rehab Services department in Muskegon, MI and does provide physical therapy services on an as-needed basis at Shelby Hospital in Shelby, MI.

Position Purpose:
The physical therapist will assume autonomous responsibility for evaluating patient’s need for physical therapy.
What the Physical Therapist ( PT ) Will Do:
-
Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.

Evaluation and clinical diagnosis of patient conditions.
-
Establish treatment goals utilizing input from patient and their family as appropriate Documentation remains in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.

Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan
Minimum Qualifications:

Must be a graduate of a Doctorate’s or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.

Required: Bachelor’s in Physical Therapy

FSBPT Certification.
-
State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.

American Heart Association BLS or equivalent
