Key Responsibilities

  • Build and maintain strong relationships with medical professionals, hospitals, and community healthcare partners

  • Represent and promote the organization’s mission, philosophy, and services both within hospitals and throughout the community

  • Attend facility meetings, calls, and educational in-services

  • Maintain accurate and up-to-date referral tracking logs

  • Provide timely end-of-day reports to department heads

Qualifications

  • Valid driver’s license required; travel is a key component of this role

  • RN, LPN, or LSW preferred, but not required

  • Ability to manage multiple residents and referrals in a fast-paced environment
