Schedule: Full time Shift: Day (United States of America) Address: 601 E ROLLINS ST City: ORLANDO State: Florida Postal Code: 32803 Job Description:

  • Supports complex patient care issues, identifies patients at risk for suboptimal outcomes, and proposes proactive care plans.
  • Acts as a liaison with the Hepatology patient population, the Liver Transplant Coordinator, and community care providers.
  • Maintains knowledge of current evidence-based practices in area of expertise. Advocates for Hepatology patients as an active member of the Hepatology team, including advanced assessment, planning, coordination of care, modification of treatment plans, and monitoring and evaluation of outcomes.
  • Assists and guides patients with non-medical needs related to care insurance/referral processes, pharmaceutical forms and prescriptions, and disability/FMLA forms.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • Strong assessment skills [Required]
  • Sound clinical judgement, decision making, and problem-solving skills [Required]
  • Concise oral and written communication skills [Required]
  • Ability to work independently, as well as, within a team environment [Required]
  • Basic computer skills [Required]
  • Bilingual (English/Spanish) [Preferred]

Education:

  • Bachelor's [Preferred]

Field of Study:

  • in a health-related field

Work Experience:

  • Experience with participation in the care of patients with liver/GI disease, organ failure or chronic illness [Preferred]
  • Patient care management in the outpatient setting experience [Preferred]
  • Teaching/preceptor experience [Preferred]
  • Three years nursing experience in inpatient and/or outpatient [Required]
