Job Summary

Responsibilities

Provide high-quality patient care to our residents, including administering medications, wound care, and treatments as prescribed by physicians

Develop and implement individualized care plans for each patient, taking into account their unique needs and preferences

Conduct initial and ongoing assessments of patients' physical and emotional needs, identifying opportunities for improvement and implementing strategies to address them

Collaborate with other members of the care team to ensure seamless communication and coordination of patient care
