MAIN FUNCTION
-
Oversee discharge/continuing care planning for patients transitioning to Seafield sober homes.
-
Ensure smooth transition to next level of care.
-
Manage user role(s) and system access as assigned.
-
Essential Day 2 for emergency closure.
ESSENTIAL DUTIES AND RESPONSIBILITIES
-
Ensure that all clients are treated with dignity and respect.
-
Affirming care for Lesbian, Gay, Bisexual, Transgender and Questioning clients.

Conduct discharge planning and document in EMR.
-
Coordinate transportation and referrals to services.
-
Liaise with case managers and next care facilities.
-
Schedule outpatient appointments and telehealth sessions.
-
Manage Seafield sober home placements, admissions, and discharges.
-
Maintain house census, Resources balances, and weekly reports.
-
Screen calls and evaluations for housing criteria.
-
Perform office duties and maintain communication with house managers.
-
Complete monthly housing reports and other tasks as assigned.
COMPETENCY REQUIREMENTS

Knowledge of referral sources

Individual counseling
-
Discharge and referral coordination

EMR documentation
-
Proficient in EMR, Microsoft Outlook, and other systems

Follow-up calls
EDUCATION AND QUALIFICATIONS
-
Bachelor’s Degree preferred
-
Minimum one year of discharge planning experience
-
CASAC and supervisory experience preferred
PHYSICAL DEMANDS
-
Requires talking, hearing, and specific vision abilities (close, distance, and focus).
-
Primarily sedentary with occasional walking and standing.
-
Must be able to use a computer, keyboard, mouse, calculator, copier, fax, scanner, and phone.
