Job Description

Are you a licensed nurse passionate about individualized care and community inclusion? Homeward Bound is hiring a dedicated Nurse Case Manager to support individuals with developmental disabilities in leading full and dignified lives. This position blends clinical practice with leadership and advocacy in a person-centered environment.

As a Nurse Case Manager, you will coordinate and oversee the healthcare of individuals served by Homeward Bound. This includes developing health care plans, facilitating care with medical providers, ensuring compliance with health regulations, and mentoring direct care staff.

Key Responsibilities

  • Assess, plan, and monitor individualized health care needs

  • Order and track medications, medical supplies, and adaptive equipment

  • Ensure compliance with DHS, ICF, and Homeward Bound policies and procedures

  • Complete quarterly and annual health evaluations and present at IDT meetings

  • Train unlicensed staff and ensure competency in delegated nursing tasks

  • Provide emergency response and participate in on-call rotation

  • Serve as a liaison with families, guardians, and external healthcare providers

  • Maintain accurate documentation and participate in investigations as required

Requirements

  • Active RN license in the State of Minnesota

  • CPR and First Aid certification (or willingness to obtain prior to hire)

  • Valid MN driver’s license and acceptable driving record

  • One (1) year of experience with individuals with developmental disabilities and complex medical needs

  • Ability to lift and transfer up to 60 lbs

  • Completion of a Minnesota Department of Human Services background check

Preferred Skills

  • Strong communication, documentation, and critical thinking skills

  • Knowledge of the Nurse Practice Act and DHS/ICF regulations

  • Ability to work independently and within a collaborative team
