Job Description

Employment Horizons is a nonprofit organization dedicated to assisting people with barriers to employment in achieving their individual vocational objectives and establishing self-sufficiency in the community.

Position Overview:

As the Community Transition Specialist, you'll help people with disabilities take the next step from Vocational Rehabilitation Services into real jobs in the community. You're a natural relationship-builder; someone who can walk into a local business, make the case for hiring one of our participants, and follow through to ensure it's a great fit for everyone involved. You'll match people to the right work, train them once they're on the job, and keep checking in with participants and employers to make sure it's working.

Schedule: Full-time, Monday through Friday, 8:00 AM – 4:00 PM. Occasional nights and weekends as needed to support participants or employers.

Key Responsibilities:

Build relationships with local businesses to create job exploration, training, and placement opportunities for Extended Employment participants

Match participants' skills, interests, and support needs to the right job or work experience, in partnership with Vocational Rehabilitation staff

Conduct job sampling with participants to help them explore different types of work before committing to a placement

Assess community job sites for safety and fit before placing participants

Help participants search and apply for jobs, and prepare them for interviews through coaching and mock interviews

Accompany participants to job interviews and provide on-site support

Provide transportation or travel training to job interviews and worksites as needed

Train participants on the job and coach them through performance and workplace adjustment

Gather ongoing feedback from employers to ensure participants are meeting job expectations

Educate employers on accommodations, assistive technology, disability awareness, and the ADA

Maintain accurate service documentation, case notes, and reports

Qualifications

Required Qualifications:

Bachelor's degree in Psychology or a related field

2+ years of experience providing supported employment services to people with disabilities

Attended, or willing to attend, Boggs Center Employment Specialist Foundations training

Strong presentation skills, comfortable speaking to individuals and groups of all sizes

Comfortable cold calling businesses

Excellent time management, conflict resolution, and negotiation skills

Excellent interpersonal and customer service skills

Strong organizational and multitasking skills

Proficiency in Microsoft Office, including Word, Excel, and PowerPoint

Excellent listening, oral, and written communication skills

Ability to work independently, with sound judgment and common sense

Valid driver's license

Preferred Qualifications:

Experience cold calling businesses

Physical Demands & Work Environment:

This position requires the ability to drive, walk between locations, and occasionally support participants outdoors or in varied work environments. The employee is regularly required to talk and hear, and frequently use hands to type or handle objects. They may occasionally lift up to 25 pounds. Vision requirements include close, distance, peripheral, and color vision, and the ability to adjust focus. The noise level may vary, depending on the job site.
