Full job description
Regular, Full-Time | Schedule: Mon-Fri 8am-4:30pm | Pay: Paygrade 13, Hourly, Depending on experience

Job Purpose or Objective(s): As the Case Manager RN, you will provide case management services and coordinates the delivery of health care for assigned patients. You will supervise other members of the health care team. You will report to the Manager or Director over the department.
Primary Tasks:

Assess and evaluate patient needs to prepare and implement treatment and case management plans.

Confer with health team members to discuss and evaluate patient information for planning, modifying, and coordinating treatment programs.

Explain procedures and treatments to patient to gain cooperation, understanding, and allay apprehension.

Administer prescribed medications and treatments in accordance with physician orders.

Prepare and maintain written case records and reports.

Manage cases to ensure that follow-up care is provided.

Coordinate and schedule appointments for consultations, referrals, and tests.

Facilitate communication between care providers.

Provide information and educate patients in various health topics.

Perform other responsibilities as may be assigned.
Required Education and Experience:

Associate degree in nursing

Current RN licensure with the Oklahoma State Board of Nursing or Multi-State RN License

Current BLS certification
