Full job description

  • Support HWD by managing Assisted Living team members.
  • Schedule Assisted Living team members.
  • Participate in Care Plan meetings with families and residents and manage ISP’s as resident needs change. 
  • Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.
  • Participate in weekly at risk meetings with HWD, ED, MCC/ MCD to ensure leadership team is aware of all resident needs.
  • Review and implement all necessary processes for optimal operational efficiency such as- shower schedules, laundry schedules, caregiver assignment sheets, etc. Level of Formal Education: HS or GED

Area of Study: General

Years of Experience: 3-5+ years caregiver experience preferred 

Type of Experience: CAN experience preferred, scheduling, communicating with families

Special Certifications: Prefer CNA

Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.

Technical Competencies: Basic computer skills, Excel for scheduling

  • Need to be organized, flexible, and even tempered.
  • Ability to communicate with families, frontline team, and community management team to ensure optimal operational success.  Personal Attributes: Calm personality that possess the ability handle stressful situations with an easy demeanor, ability to multi task and follow through, and self-motivated.
