Position Summary: Case Manager (MLTSS/D-SNP Programs)
The Case Manager is responsible for developing and implementing healthcare strategies to ensure quality care for members, particularly those in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE).
Key duties include:

Conducting in-home assessments using comprehensive tools.

Creating and managing individualized care plans.

Coordinating services such as adult/pediatric daycare, personal care, and nursing facility placements.

Collaborating with members, families, providers, and interdisciplinary teams.

Ensuring cost-effective service delivery and accurate documentation.
-
Mentoring new hires once proficient.
-
Traveling 50–75% within assigned counties.
Skills Required: Advanced clinical judgment, critical thinking, strong communication, and assessment skills. NJ Choice Certification is required.
Required Qualifications

Active and Unrestricted RN license in NJ required.

Must reside in Somerset County NJ
-
3+ years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.

Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy

Pay Range
The typical pay range for this role is:
$66,575.00 - $142,576.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
